When compared with no treatment, water, hydrogen peroxide and olive oil had a clinically beneficial effect but there was no clinically important benefit of sodium bicarbonate in facilitating immediate irrigation. There are several different types of ear drops. The one that’s right for you depends on a few factors, including your symptoms and what type of ear condition you have. Ear drops for ear infections It is recommended you use a dropper while lying your head on one side for a few minutes to let the oil work its way through your ear canal(s). Although less commonly known, ear candling may still be requested by some patients. This should never be used in the management of ear wax removal and may result in serious injury [9] This will require an urgent GP referral for antiviral medication, either with or without oral steroids [11], [12]

opening through the eardrum (e.g. perforated eardrum or fitted ventilation tube called a ‘grommet’) A dry ear is unlikely to become infected, so be sure to dry your ears thoroughly after swimming or bathing. Irrigation of the ear canal should be undertaken using warm water and an electronic ear irrigation machine with a variable pressure control. A jet tip is used to angle the flow of water along the top of the posterior wall. Historically ear syringing was used and this is evident in the papers reviewed. The effect is the same but irrigation is a much safer method and syringing is now contraindicated because of adverse effects, namely trauma. The outcomes identified as critical outcomes for this review were health-related and hearing-related quality of life, any outcomes related to wax removal, for example ability or ease of removal, and adverse events such as perforations or infections.

EAR WAX DROPS & SPRAYS

They may suggest using eardrops for a bit longer, or they may carry out a minor procedure called ear irrigation (previously syringing) to clean out your ear canal. Not all GP practices offer this service. You may have to pay to have this treatment privately.

Don’t try to scrape out the earwax with your finger or an object inserted into your ear, as this this can make the problem worse. When different ear drops were compared with each other, chlorobutanol was found to have a clinically important benefit compared with almond oil and to sodium bicarbonate. Both chlorobutanol and sodium bicarbonate showed clinically important benefits compared with water and hydrogen peroxide urea was more clinically beneficial than chlorobutanol. Acute otitis media (inflammation of the middle ear)and ‘glue ear’ can affect one or both ears in children. In adults, both ears being affected may be a sign of age-related hearing loss; however, if only one ear is affected, further investigations may be required to exclude other causes, such as an acoustic neuroma (a benign growth on the hearing and balance nerve).

The committee agreed that the critical outcomes for decision-making were successful earwax removal; improvement in hearing; adverse events related to earwax including perforation, infection, vertigo, bleeding and discomfort; hearing-specific health-related quality of life as reported by validated scales including WHO DAS II, HUI2/HUI3, Cambridge Otology QoL Questionnaire and the SSQ. Further critical outcomes included patient reported disability or benefit; measures validated to demonstrate changes with audiology care in the population under study for example, the Device Orientated Subjective Outcome Scale, Glasgow Hearing Aid Benefit Profile and Hearing Handicap Inventory for the Elderly. Patients should be warned that the drops may cause transient hearing loss, discomfort, dizziness and skin irritation. Drops should not be advised in patients known to have a perforated eardrum, with a history of ear surgery (unless advised by the ENT specialist) or if an infection is present.
