The next stage was to investigate whether CDED can be used to treat adults with CD, patients with severe luminal CD, therapy for longer than 12 weeks and for patients refractory to biologics and/or as an adjuvant therapy to biologics.
In another randomized controlled trial 25, 44 adults with mild to moderate CD were treated with CDED. Patients were randomly assigned to CDED plus PEN (20) or CDED alone (24) for 24 weeks. Another mechanism of dietary interaction with the host is via a direct effect on the immune system, where dietary antigens by themselves may trigger an immune response. The typical western-style diet, high in sugar and saturated fats and low in fibre, can lead to systemic low-grade inflammation, as described in obesity. 25 In animal models, the consumption of milk-derived saturated fats alters bile acid composition, allowing for a bloom of sulphate-reducing bacteria, which in turn can produce greater amounts of the potentially mucosal toxic hydrogen sulphide and production of helper T cell (Th1) proinflammatory cytokines. 26

This protein is significant, since it can protect TGF-β2 from duodenal enzymatic degradation [ 7]. Potential beneficial effects may be due to the whole protein or to its derived peptides ( Figure 1, right panel). In an ileitis model, macroscopic and microscopic lesions, and Goblet cell depletion were protected by β-casofensin [ 46]. The amino acid profile of casein proteins is principally rich in two essential amino acids and one non-essential [ 47]. The first one is leucine (from 69 to 108 mg/g), which promotes cryptidin-1 production by Paneth cells via Slc7a8 transporter [ 48]. The second one is lysine (from 49 to 67 mg/g), which has anti-inflammatory properties as demonstrated by the reduction of weight loss, disease index, and inflammatory cytokines in dextran sulfate sodium (DSS) induced colitis [ 49]. Finally, glutamic acid presents the highest concentration (from 218 to 239 mg/g). This amino acid has been widely studied in the intestine and it is recognized as a principal actor in intestinal integrity ( Figure 1, right panel). Not only glutamic acid can regulate proliferative, apoptotic, and inflammatory cellular pathways, but also tight junction proteins [ 50]. Glutamic acid can act directly on proteins such as ERK1/2, STAT, and HSF, and indirectly by enhancing growth factors’ effects like EGF and TGF-α ( Figure 1, right panel). The past few years have seen an expansion in IBD therapeutic options. Conventional treatments control symptoms through pharmacotherapy, including aminosalicylates, corticosteroids (CSs), immunomodulators, and biologics, with other general measures and/or surgical resection if necessary. The introduction of specific inhibitors of tumor necrosis factor (TNF) is a groundbreaking achievement, enabling long-standing remission, and modification of the IBD course in a significant fraction of patients ( 5). However, primary non-response to TNF inhibitors was observed in up to 40% of patients in clinical trials and 10–20% patients in clinical series; secondary loss of response occurred in ~23–46% of patients after 1 year of treatment ( 6), which calls for new therapeutic strategies. New therapeutic strategies are emerging, involving small molecules, apheresis therapy, improved intestinal microecology, cell therapy, and exosome therapy. In addition, patient education on diet and psychology appears to benefit IBD treatment.
Finally, in addition to gut integrity, Modulen ® elements will likely benefit other organs and physiological processes allowing a well-being stage. Naturally, physical ameliorations go along with mental ones, thus achieving an effective quality of life. In a 12-week prospective trial of children with mild to moderate CD conducted in 2019 1, children were randomly assigned to a group that received CDED plus 50% of calories from partial enteral nutrition (PEN, Modulen IBD) for 6 weeks (phase 1), followed by CDED with 25% PEN from weeks 7 to 12 (phase 2). The Palaeolithic diet 59 is based on the idea that the human intestinal tract is not sufficiently evolved to digest food from modern agriculture and thus, the exposure to foods not present at the time of human evolution may cause modern diseases, such as IBD. In common with other diets, the Palaeolithic diet excludes many food ingredients and privileges the intake of lean, nondomesticated meats and noncereal plant-based foods (i.e. fruits, roots, legumes, and nuts). So far, no data exist on the role of the Palaeolithic diet in the treatment of IBD, except of rare and isolated rather positive case reports. Following the studies, it was felt that UCED combined with PEN would provide complementary nutritional support and improve outcomes.
The SCD limits or totally excludes grains, fiber, and certain sugars, with the idea that undigested starch products may trigger changes in intestinal bacterial growth and inflammation. The SCD eliminates all refined and processed foods (e.g. processed meats, artificial sweeteners), along with soy, lactose, sucrose, and grains. Certain vegetables are also taken out of the diet, including potatoes, okra, and corn. Other than intestinal symptoms, CD can lead to extra-intestinal ones such as bone, skin, ocular, and thromboembolic complications. These events were not investigated after EN and more specifically Modulen ® therapy. This goes along with other organ consequences. The most alarming repercussion is steatosis as non-alcoholic fatty liver disease is common in IBD. Considering TGF-β2 content, and the risk of hepatic fibrosis, hepatologists could avoid Modulen ® therapy for CD patients.
A short pilot study reported symptomatic improvement through excluding foods with high immunoglobulin (Ig)G4 levels (mostly eggs and beef). 64 MRI showed a reduction in abnormality from 2019. The patient only required EEN for two days, had gained 4.6 kg since CDED, had managed well on phase 2 and moved to phase 3, was passing formed motions and did not require surgery.
