To mitigate the risk of an ETT‐inflicted injury, manufacturers recommend that an ETT should not be secured to place (i.e. by means of a tube‐fastening device) in patients who have facial oedema, lip oedema, or protruding teeth. Moreover, it has been reported that increasing the frequency of ETT repositioning from 12hours to 4 hours decreased the prevalence of ETT‐related PUs from 16% to 10%. Nonetheless, this reduction was not statistically significant, which likely indicates that ETT repositioning per se is insufficient for effective prevention of ETT‐caused PUs. 20 The eustachian tube has two primary functions: balancing the pressure between the middle ear and the surroundings and helping facilitate the secretions from the middle ear, as well as protecting the ear from loud and potential sound hazards. Muscles responsible for these functions are tensor veli palatini and levator veli palatini. Although they don't function similarly, their simultaneous contraction opens the tube. As the tube empties, pressure equalization between the middle ear and the atmosphere takes place. Moreover, facial movements like deglutition and yawning cause musclecontraction that is connected to the eustachian tube, allowing a small amount of air into the tube [ 2]. However, other functions like aiding in draining the middle ear secretions and their clearance from the ear cavity[ 3]. Oddly, we also seem to prefer an early morning facial, with high search spikes for the phrase between 4am and 5am on Google this past week.

Other devices, such as the laryngeal mask airway (LMA) or the double lumen esophageal-tracheal Combitube, can be inserted blindly and do not require view of the vocal cords by any means. Another option for endotracheal intubation of a patient with maxillofacial injury is to place an LMA and then pass an endotracheal intubation tube through the LMA. The final option is the surgical one: to establish a direct access to the trachea by performing a cricothyroidotomy or a tracheotomy.
